The CLPR Report

On page 3, we revisit any conditions from page 2 that had an i symbol beside them. These are conditions that can improve if the client makes the right changes.
Recommendations for change are detailed in the tan box at the top of the page.
A smaller box-within-a-box at the upper right shows the client’s current median life expectancy (the age at which the probability of survival is 50%) and beneath it shows the potential median life expectancy if the client makes the changes.
On the bottom half of the page are recommendations, again based on the client’s conditions, that may help the client maintain his or her health or improve the quality of life. However, following these recommendations is unlikely to change the survival probabilities.
Beneath the potential life expectancy box is a reminder that life expectancy can clearly change. It is wise to re-evaluate every 2 to 3 years. 21st Services offers a discount for any client’s subsequent CLPRs.
