Ordinarily, you would supply 21st Services with copies of your medical
records. However, 21st Services will handle all communication with physicians and obtain the medical records
for a small fee plus the copy cost charged by the physicians office.
Every Customized Longevity Planning Report - or CLPR - is based on the information
we gather about your specific medical, family and personal history. With the Standard
CLPR, the analysis is based on your medical records for the past five years.
The records are reviewed by our underwriting staff and key factors are analyzed
by our sophisticated proprietary software system, built on our extensive mortality

The CLPR report includes your longevity curve, compared to a "standard" mortality
curve, and the ages at which your survival probability is 50%, 30% and 10%.
